Our plan is to do what works: promoting best-practice care models to ensure high-quality, seamless care for patients and families. One way we’re ensuring that what works continues to work is by designing our models of care to become better with age, equipped with systems to measure and constantly improve care outcomes.
As our country moves toward providing better care for those with advanced illness, we will continue to ensure that the public stays empowered and health professionals stay supported — so that our fellow citizens don’t lose the good care for which they fought so hard.
